What about FMLA?

So I hear you’re having surgery? That’s wonderful. Getting on the schedule is a strong step toward recovering your quality of life, and we’re glad you’ve chosen to trust us in this process.


But what if you work? You can’t just disappear from work from several weeks to recover with notifying your work and getting permission.


Sounds like you’ll need to get some FMLA or short-term disability paperwork filled out. Your employer can provide you with a copy of the paperwork, but what then? Let me explain.


First of all, there is typically a portion of the paperwork that your employer needs to fill out. Make sure to consult with your HR representative to confirm that all employer sections have been completed. 


Second, it is of vital importance that the employee section is completed. Prior to giving us your paperwork to complete, please make sure to fill in your portion. If we don’t have your employee information on the paperwork, it could get misplaced or delayed. 


Finally, when we have the paperwork, we will complete it with your surgical information and in-office history. Once complete, we will send the paperwork to the indicated recipient. 


Many versions of FMLA paperwork include the HR Department or Insurance company fax/email, but we may need you to furnish us with that information. If your employer provided you an alternative or preferred fax number or email, please let us know as soon as possible.


One final note, and a very important note indeed, is the cost of FMLA paperwork. We charge a flat fee to complete any and all FMLA, short term disability, return to work, etc. paperwork associated with each surgery. Please call our office to discuss any questions about your FMLA paperwork.
